Registered Nurse

Hi!
I am a Registered Nurse and have started a business providing consulting and bedside
advocacy/support to patients that are dealing with Endometriosis and Endometriosis Excision
Surgery.

I have a client that I will be providing bedside advocacy/care for at the end of December. I am
looking for a contract template that I can use that will be legally binding and that I can use to
plug in client’s names and prices, dates, etc as I move forward.
I WILL NOT be providing medical care per se…. my job is to be there to be an
advocate/liaison/consultant. I will be in working in cooperation with the patient’s treatment team,
including surgeon, surgery center, neurologist, physical therapist, etc. If there is a way to include
language that includes a waiver against liability in terms of malpractice that would be great.
I would also be okay if I need to purchase two separate contracts…. One payment contract for
services and a separate waiver for malpractice.

Does this make sense?
